- Date of MVA
- Location (as precise as possible) of MVA
- Names, addresses and contact information of all drivers and owners of the vehicles involved
- Plate numbers of all vehicles involved in the MVA
- Vehicle damage estimates for all vehicles involved, if available
- Names, addresses and contact information of any passengers
- Names, addresses and contact information of any witnesses to the MVA
- Copy of accident report
- Photographs of vehicles involved, scene of accident and visible injuries
- Diagram of where and how MVA occurred
- Claim Number
- Receipts
- Medical information
- Employment information
- Photo ID
- Care Card number
- SIN
